Driver Authorization Form

To obtain authorization to drive a university insured vehicle, please complete the Driver Authorization Form below. The Driver Authorization form is also available in the office of Risk Management and Insurance located in Flynn Hall.

We request three (3) days to complete the process. Please do not rely on immediate approval.

After the three day waiting period, unless you are informed otherwise, you may assume that you are approved to drive a university insured vehicle. Please check the list of authorized drivers posted on the Risk Management website. For those instances when driver approval is not granted, the authorizing supervisor named on the form will be notified by the office of Risk Management and Insurance. If you are uncertain of your approval, after the three day period you may call the office of Risk Management to check on status of your request.

If you wish to drive a van, please inquire about defensive driving training sessions specifically for use of a 12 or 15 passenger van. Anyone who will drive a 12 or 15 passenger van must attend mandatory van driver safety training.
