Assumption of Risk Waivers

Each club sport athlete is required to complete an Assumption of Risk Waiver prior to their first practice. Our electronic waivers are specific for each club sport, so if you are in multiple clubs, you will need to complete multiple waivers --> Complete Assumption of Risk Waiver: Log in to EngageXU

When you are on the home page, click the search option in the top right. Type in Recreational Sports and click to enter that portal. On the Recreational Sport home page, click on the "Surveys and Forms" button. Then click on the "Assumption of Risk 101 - On-Going Experience (Club Sports" link. From here you will complete the form. 

 

In addition to completing this online waiver, participants must provide an annual physical clearing them for activity.
