Planning For Your Appointment
How to schedule an appointment:
If you are a new or returning patient, schedule an appointment by contacting Xavier University Health Services by phone at 513.745.3022 opt. 3.
We ask for your consideration and cooperation in scheduling and canceling appointments. If you cannot keep your appointment or going to be late, please contact our office at 513.745.3022 opt. 3 as this may affect availability of care to your peers here at Xavier University.
Scheduling tips
- When scheduling your appointment please inform us of all your symptoms and healthcare needs when obtaining your visit so that the appropriate amount time may be allotted to address your concern(s).
- Allow at least an hour for your appointment.
- We advise that you call early in the day if you think you will need a same day appointment.
- Have your XU One Pass and insurance card available when scheduling your appointment and bring a physical copy with your for your appointment.
Preparing for your appointment:
In preparation for your appointment please ensure you have the following prior to arriving at the Health Services office.
- A physical copy of your insurance card and form of identification (XU One Pass or Driver's License) for verification.
- If using your parent's insurance, you will need to know the subscribers full name, the name of their employer, and date of birth.
- A copy of your COVID-19 Vaccine Card (Recommended for entry into your medical record)
- A form of payment for possible co-pay or if you are prescribed a medication.
- We accept cash, credit/debit card, or XU One Pass
- Any paperwork you need filled out including physicals, screenings or immunization documents
- Check with your program or employer if specific documents are needed.
Arriving to your appointment:
Please arrive to your appointment 15 minutes early to allow enough time to check-in with one of our registration specialists.
- You will be asked to state your name, date of birth, and time of appointment.
- We will request a physical copy of your insurance card and form of identification (XU One Pass or Driver's License) for verification.
- If using your parent's insurance, you will need to know the subscribers full name, the name of their employer, and date of birth.
- An involvement of care form will be given to you and explained during your appointment.
- Depending on your insurance, a co-pay may be collected. We accept cash, credit/debit card, and XU One Pass.
Our medical team will request you complete a brief survey following your visit to ensure we are providing exceptional quality care and meeting your health care needs.
