Tutoring Intake Form

Contact Stephanie Daniels at danielss3@xavier.edu or 513-745-3214 if you need help connecting with a tutor or scheduling an appointment.

You may request a maximum of 45 minutes of tutoring per class, per week.  You will schedule your first appointment through the system, if you want the appointment to be weekly, you can talk to your tutor or you can email oas@xavier.edu to schedule it weekly.

Course-Specific Tutoring Requests
