Voluntary Benefits
In addition to the Medical, Dental, Vision, Tax Advantage Accounts and employer provided Life Insurance and LTD, Xavier also provides the opportunity for employees to purchase additional insurance coverages. These include Voluntary Life and AD&D, Accident and Critical Illness.
Voluntary Life and AD&D
Voluntary Life and AD&D allows an employee to purchase additional life insurance coverage above the Basic Life and AD&D amount provided by Xavier. Employees may elect coverage for themselves, their spouse and/or their dependent children up to age 26. The Guarantee Issue (GI) and Maximum Elections amounts are:

- Evidence of Insurability (EOI) is required for elections above the GI amount.
- Benefit amount reduces to 65% of original coverage when employee reaches age 65 and 50% at age 70.

Accident
Accident coverage can help offset expenses for treatment of covered injuries due to an accident. Employees must submit a separate claim form to the carrier in order to receive the benefit. If approved, the check will be mailed to the employee's home address for the benefit amount. Example claims and the benefit amount are:

Critical Illness
Critical Illness coverage can help to offset expenses incurred if you or a covered family member are diagnosed with a covered illness. A flat benefit amount is paid by check to the employee to use for healthcare or personal expenses. Employees may elect coverage for themselves, their spouse, and/or their dependent children up to age 26. Employees may elect between $5,000 and $30,000 in $5,000 increments. The spouse coverage is 50% of the employee election and child(ren) coverage is 25% of the employee election. Example claims and the benefit are:
